Live Well Health Guide provides concierge medical, dental, wellness, and related health services. Our services are intended for adults 18 years of age and older. All medical decisions remain the sole discretion of licensed physicians.

2. Appointment Policy

Appointments must be cancelled at least 24 hours in advance. Cancellations made with less than 24 hours notice may incur a cancellation fee. Repeated no-shows may result in termination of membership or access to services.

• Arrive 10 minutes before scheduled appointment time
• Bring valid government-issued photo identification
• Bring your insurance card if applicable
• Complete patient intake forms prior to your first visit

3. Payment Terms

Payment is due at the time of service unless a prior billing arrangement has been established. We accept all major credit cards, FSA/HSA cards, and approved insurance plans. Membership fees are billed monthly or annually as selected and are non-refundable after 14 days of the billing cycle.
